急症内镜检查对132例老年上消化道出血的诊断价值Emergency Endoscopy for the Diagnosis of Geriatric Acute Upper Gastrointestinal Bleeding

摘要(Abstract)：
本文分析了急症内镜对老年上消化道出血的诊断价值,并探讨该病的病因。结果表明,老年上消化道出血的急症内镜检查能及时明确病因,诊断准确率达94.9％,且受检时间与检查的阳性率呈正相关。本组老年上消化道出血的常见病因以胃溃疡居首位,其它依次为十二指肠溃疡、胃癌、急性胃粘膜损伤及肝硬化。
Emergency diagnosis gastroscopy was performed on 132 old patients for acute upper gastrointestinal bleeding, with 94,9% (125/132) diagnositic accuracy.There were the positive correlation between diagnositic accuracy and the time of endoscopic examination (r=0,311,P<0.01). Gastric ulcer was the cause of bleeding in 25% of the cases, duodenal ulcer in 22.7%, gastric cancer in 14.4%, acute gastric mucosal lesion in 10.6%, and cirrhosis of liver with bleeding in 5.3%. Of the 132 patients, male patients with gastric ulcer and cirrhosis of liver were much more than the female(P<0.05). No significant statistical difference was found betweeu male and female patients with duodenal ulcer and gastric cancer. These results suggest that emerygency endoscopy be a safe and trustworthy diagnositic method in the diagnosis of geriatric acute upper gastrointestinal bleeding.
