红景天预处理拮抗肝叶切除患者肝脏的缺血-再灌注损伤
Pretreatment with Salidroside against Hepatocyte Ischemia-reperfusion Injury in Partial Hepatectomy Patients

摘要(Abstract):

目的:观察红景天预处理对肝叶切除患者肝脏缺血-再灌注损伤的保护作用。方法;64例拟行肝叶切除的患者随机均分为对照组(A组)和红景天组(B组),B组大珠红景天胶囊连续口服5 d,最后一次于术前30 min以大珠红景天注射液加生理盐水100 ml静脉滴入,A组静脉滴入等量的生理盐水;在肝门阻断再灌注1h、再灌注6 h及再灌注24 h时抽取静脉血测定谷丙转氨酶(ALT)、谷草转氨酶(AST)及丙二醛(MDA)水平,并于再灌注1 h取肝组织进行病理检测。结果:A组和B组ALT再灌注1 h,再灌注6 h及再灌注24 h 3个时相点均升高,再灌注6 h达到峰值,但B组升高幅度较A组低,差异有统计学意义(P<0.05);AST在3个时相点也升高,于再灌注6 h达到高峰并与ALT一致;MDA 3个时相点B组升高幅度显著低于A组,差异有统计学意义(P<0.05)。再灌注1 h肝组织损害B组较A组减轻。结论:红景天预处理能抑制氧自由基的产生,对肝脏缺血-再灌注损伤具有保护作用。
Objective: To investigate the protective effects of salidroside pretreatment on hepatocyte functions after ischemia-reperfusion injury in partial hepatectomy patients.Methods: Sixty-four liver tumour patients were randomly divided into a saline control group(A),and a salidroside pretreatment group(B).The patients in group B were pretreated with salidroside orally for five days,and the last administration was at 30 minutes before operation by vein injection.Patients in group A were pretreated with normal saline at the same dose and time.During the operation of partial hepatectomy,hepatic blood flow was totally clamped for 10~30 min and then released.In 1h,6h,and 24h after reperfusion,levels of alanine aminotransferase(ATL),aspartaste aminotransferase(AST),and malondial dehyde(MDA) were detected.Liver tissue at 1h after reperfusion was taken and examined on common paraffin sections with HE staining.Results: The serum levels of ALT,AST,and MDA were markedly lower in group B than those in group A(P<0.05) at time points of 1h,6h,and 24h after liver reperfusion.Liver tissue injury and hepatocyte damage in group B were milder than those in group A at 1h after reperfusion.Conclusions: Pretreatment with salidroside could inhibit production of oxygen free radicals,which has some protective effects to liver tissue against hepatocyte injuries caused by ischemia-reperfusion.
