体外循环期中血及电解质变化CHANGES IN BLOOD-GAS TENSION AND SERUM ELECTROLYTE DURING HYPOTHERMIC EXTRACORPOREAL CIRCULATION

摘要(Abstract):
回顾性分析50例低温体外循环心内直视手术的血气及血清电解质变化。据测定结果认为:氧合正常,一般不会发生低氧血症;转流中的主要问题是呼吸性碱中毒;转流未使血清钠进一步下降;低钾血症的原因可能为pH升高、低温及多尿。为了提高心脏自发性复跳的百分率,在整个麻醉及手术过程中必须补充氯化钾。
This paper presents the changes in blood-gas tension and serum electrolytes during open heart surgery with hypothermic extracorporeal circulation. Fifty cases from Sep 1982 to Aug 1986 were reviewed retropectively. Blood samples were taken before CPB (Cardiopulmonary bypass), at 10-15 minutes after the onset of CPB and at the end of CPB. From the measuring data we concluded; 1. Oxygenation was normal, hypooxemia didn't occur usually. 2. Respiratory alkalosis was the principle problem during bypass. 3. Perfusion didn't lead to further becreasing in serum sodium. 4. The cause of hypopotassemia might be rising pH, hypothermia and polyuria. In order to obtain a heigher percentage of spontaneous heart contraction Potassium Chloride must be given in the whole course of anesthesia and operation.
