美吡达的降糖机理及对胰岛、垂体、甲状腺、肾上腺皮质功能影响的探讨

摘要(Abstract):

用美吡达治疗22例新发Ⅱ型糖尿病,总有效率为81.9%。治疗后患者的空腹、餐后2h和馒头餐负荷下的血糖及血糖面积明显降低,胰岛素释放试验结果、胰岛素面积、胰岛素生成值明显升高,胰岛素耐受试验结果明显改善,血清T3、FT3水平增高,PRL水平降低。本文表明,美吡达降糖作用肯定,除能刺激胰岛β细胞分泌胰岛素和加强机体对胰岛素的敏感性外,还可能改善胰岛β细胞以外的内分泌功能紊乱。
In this study 22 patients with diagnosed type Ⅱ diabetes for the first time were treated with glipizide and the effective rate totally was 81.9%.The blood glucose of the effective cases at fasting and 2 hours after meals well as during steamed bread test and blood glucose areas were significantly decreased after treatment. In steamed bread stress test,insulin areas and insulinogenic values were increased significantly after treatment.Also,the blood glucose level was much lower and six of 18 effective patients (33.3%) even had hypoglycemia during the insulin tolerance test. After treatment serum TT3 and FT3 levels were elevated but serum TT4 and FT4 levels did not change.Prolactin levels were markedly reduced but there was no significant change in the levels of serum cortisol, growth hormone and glucagon. On the basis of the results it is concluded that glipizide could correct, a certain extent, disfunction of the islet-cell and other endocrine glands besides its hypoglycaemic action by stimulating β-cell to secret insulin and increasing sensitivity to insulin.
