心房纤维性颤动对心功能的影响——B型超声心动图分析研究

摘要(Abstract):

应用B型超声心动图结合M型超声心动图研究了42例房颤病人的心功能。通过对EDV、ESV、SV、CO、EF%、△D%及mvcf的研究,发现心率是影响房颤心功能的主要因素,随着心率的增加,EDV、SV及EF值呈线性降低。在风心病二尖辦狭窄病人最大二尖辦口面积对EDV及SV有轻度影响。恢复窦性心律后,心功能的改善尚与心肌收缩力的加强有关。
The cardiac function in 42 cases (18 males and 24 females) with atrial fibrillation was studied with two-dimensional echocardiograms and M-echocar- diograms. The data of cardiac function as EDV, ESV, SV, CO, CL, EF%, D% and mvcf were analysed and calculated. Our results show that HR is a main factor affecting cardiac function in atrial fibrillation and EDV, SV, and EF% progressively decrease with HR increasing. Maximal mitral valve opning area in mitral stenosis affects cardiac function only slightly. There are no influe- nces between atrial fibrillation duration, underlying diseases and left atrial di- mensions to cardic function. Our results also demonstrate that the cardiac func- tion is obviously improved after cardioversion of atril fibrillation. The mecha- nism of the improvement of cardic function following cardioversion is not only due to the recovery of atrial fibrillation and increment of ventricular filling but also increment in myocardial contractility.
