口腔矫治器与外科手术对睡眠鼾症患者多导睡眠图、呼吸参数和糖脂代谢水平的影响Effect of Oral Appliance and Surgery on Polysomnogram,Respiratory Parameters,Glycolipid Metabolism in Snoring Patients

摘要(Abstract):
目的:比较口腔矫治器与外科手术对睡眠鼾症患者多导睡眠图、呼吸参数和糖脂代谢水平的影响。方法:94例睡眠鼾症患者按就诊时间均分为口腔矫治器组与外科手术组,口腔矫治器采用口腔矫治器治疗,外科手术组采用悬雍垂腭咽成形术治疗;治疗前、治疗后3月及1年时记录两组患者的多导睡眠图、呼吸参数、血糖、血脂及主观症状。结果:口腔矫治组治疗后1年总呼吸紊乱指数(AHI)、最长呼吸暂停时间显著低于外科手术组,最低血氧饱和度(LSa O2)显著高于外科手术组,差异有统计学意义(P<0.05);两组治疗后3月及1年时患者的空腹血糖(FBG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)及高密度脂蛋白(HDL-C)较治疗前显著降低,差异有统计学意义(P<0.05),组间比较差异无统计学意义(P>0.05);口腔矫治器组治疗后1年Epworth嗜睡评分(ESS)、Berlin睡眠症状评分(BQ)显著低于外科手术组,差异有统计学意义(P<0.05)。结论:口腔矫治器与外科手术均可有效改善睡眠鼾症患者症状,提高血氧饱和度,改善糖脂代谢异常情况,提高患者生存质量,其中口腔矫治器对睡眠鼾症患者主观症状改善作用更持久稳定。
Objective: To compare the effect of oral appliance and surgery on polysomnogram,respiratory parameters,glycolipid metabolism in snoring patients. Methods: 94 snoring patients were equally divided into the oral appliance group and the surgery group according to the treatment time. The oral appliance group were treated with oral appliance while the surgery group were treated with uvulopalatopharyngoplasty( UPPP). The polysomnogram,respiratory parameters,blood glucose,blood lipids and subjective symptoms of the two groups were recorded before treatment and 3 months and one year after treatment. Results: One year after treatment,the total apnea hyponea index( AHI) and the longest apnea time in the oral appliance group were significantly lower than those in the surgery group( P <0. 05),and the lowest blood oxygen saturation( LSa O2) was significantly higher than that in the surgery group( P < 0. 05). The fasting blood glucose( FBG),total cholesterol( TC),triglyceride( TG),low density lipoprotein( LDL-C) and high density lipoprotein( HDL-C) in the two groups 3 months and one year after treatment were significantly lower than those before treatment( P < 0. 05),without statistical difference between the two groups( P > 0. 05). The Epworth sleepiness score( ESS)and Berlin sleep symptom scores( BQ) of the oral appliance group one year after treatment were significantly lower than those of the surgery group( P < 0. 05). Conclusion: Both oral appliance and surgery can effectively relieve symptoms,increase blood oxygen saturation,improve glycolipid metabolism and quality of life of snoring patients.
