摘要(Abstract):

目的:了解40岁及以上不同糖代谢男性人群臂踝脉搏波传导速度(ba PWV),分析不同糖代谢水平与ba PWV的相关关系。方法:调查贵阳市云岩区40岁及以上成人男性共1 167人,同时进行问卷调查、体格检查、相关生化指标及ba PWV的测定,据血糖水平分为正常组(388人),空腹血糖受损组(IFG组,117人),糖耐量减低组(IGT组,319人),糖尿病组(DM组,343人);比较各组ba PWV水平,分析ba PWV的影响因素。结果:IFG、IGT及DM组的ba PWV水平较正常组明显升高(P<0.05),IGT组及DM组的ba PWV水平较IFG组明显升高(P<0.05),DM组的ba PWV水平较IFG组及IGT组明显升高(P<0.05);多因素Logistic回归分析发现,餐后血糖升高、空腹血糖升高、收缩压及舒张压水平增高、高龄是ba PWV增高的重要影响因素,尤其餐后血糖升高对动脉粥样硬化的影响更大。结论:男性ba PWV的水平随着糖代谢紊乱程度的加重而逐渐升高,控制血糖水平,尤其是餐后血糖水平,对预防男性心血管疾病的发生具有重要意义。
Objective: To investigate the levels of brachial ankle pulse wave velocity( ba PWV) of man aged 40 years old and above with different glucose metabolism level,and to analyze the correlation between different glucose metabolism level and ba PWV. Methods: Totally 1 167 male subjects from Yunyan district of Guiyang city were enrolled in this study. All subjects underwent questionnaire,physical examination,related biochemical indexes detection and ba PWV determination. All subjects were divided into four groups according to glucose level as following: normal control group( normal glucose tolerance,388 cases),IFG group( 117 cases),IGT( 319 cases) and DM group( 343 cases). The levels of ba PWV were compared between each group,and influential factors were analyzed.Results: The levels of ba PWV in IFG,IGT and DM group were significantly higher than those in normal control group( P < 0. 05). The levels of ba PWV in IGT and DM group were significantly higher than those in IFG group( P < 0. 05). The level of ba PWV in DM group was significantly higher than that in IFG and IGT group( P < 0. 05). Stepwise multiple logistic regression analysis demonstrated that fasting glucose rise,postprandial blood glucose rise,high systolic and diastolic blood pressure,advanced age were important influential factors for elevated ba PWV. Especially,postprandial blood glucose rise exerted greater impact on atherosclerosis. Conclusion: The levels of ba PWV in men increase gradually with aggravation of disturbance of carbohydrate metabolism. Therefore,blood glucose control,especially,postprandial blood glucose control is significantly important for prevention of cardiovascular disease in men.
