胃网膜右动脉冠状动脉搭桥术的临床解剖学
Anatomical Aspects of the Right Gastroepiploic-Coronary Arterial Bypass

摘要(Abstract):

在37具成人尸体上,对胃网膜右动脉进行解剖观测。胃网膜右动脉血管长度平均29.8cm,其行径长度不及胃大弯一半者占10.8%;长于胃大弯一半以上者占89.2%其中长于胃大弯2/3者占21.6%。其起点、胃大弯中点处及末端外径分別平均3.3mm、2.1mm和1.3mm。解剖上模拟搭桥测试证明其长度和口径适于心脏膈面、前面和外侧面的冠状动脉搭桥。阐述了其临床解剖要点。
Anatomical investigation of the right gastroepiploic artery (GEA) was made in 37 adult cadavers. The length of the right GEA was 29.8±5.0cm (mean±SD).Compared with the length of the greater curvature of the stomach, it was longer than one second of the latter in 89.2% of the cases, longer than two thirds of the latter in 21.6% of the cases, and only in 10.8% of the cases, shorter than one second of the latter. The external diameters of the right GEA were 3.3±0.6, 1.3±0.4 and 2.1±0.4mm, at its origin, distal end and middle point of the greater curvature, respectively. Anatomically mimic operation of the coronary artery bypass grafting (CABG) utilizing in situ right GEA indicated the feasibility to use the right GEA as a bypass graft to coronary vessels on the diaphragmatic, anterior and lateral surfaces of the heart. Further, surgical approachs for CABG were suggested and depicted in this report.
