摘要(Abstract)：

目的:观察腹腔镜胆道探查术(LCBDE)对胆总管结石合并胆囊结石围手术期炎性因子的影响。方法:60例胆总管结石合并胆囊结石患者,根据手术方法不同均分为治疗组(LCBDE)与对照组(开腹胆道探查,OCBDE);观察两组患者入院时、手术后当天、术后第1天及第5天清晨空腹外周静脉血白细胞(WBC)、中性粒细胞(NEUT)、超敏C反应蛋白(CRP)、白细胞介素-6(IL-6)的变化。结果:两组患者入院时WBC、NEUT、CRP及IL-6比较差异无统计学意义(P>0.05);与入院时比较,术后当天及术后第1天两组患者NEUT、CRP、IL-6均有升高,对照组高于治疗组,差异有统计学性意义(P<0.05);术后第5天治疗组患者NEUT、CRP及IL-6均低于入院时水平(P<0.05),对照组患者接近入院时水平,同时点组间比较差异有统计学意义(P<0.05)。结论:LCBDE治疗对胆总管合并胆囊结石患者的损伤程度轻。
Objective: To observe the perioperative changes of inflammatory factors,for the laparoscopic common bile duct exploration( LCBDE) and open common bile duct exploration( OCBDE) in the treatment of common bile duct stones. Methods: Sixty patients with common bile duct stones and gall bladder stones were randomly divided into treatment group( LCBDE) and control group( OCBDE)for equal number. Observing the time of admission and the day after the operation,the first day and fifth day after the operation for the WBC,CRP,IL-6 and NEUT. Results: Comparison of WBC,CRP,IL-6 and NEUT were not statistically significance on admission( P > 0. 05); but CRP,IL-6 and NEUT all rose the day and 1stday after surgery. CRP,IL-6 and NEUT levels at the 5thday after surgery were lower in LCBDE group than those in OCBDE group,the difference between groups had statistically significance( P < 0. 05). Conclusion: Compared with OCBDE group,LCBDE on the treatment for common bile duct stones shows less influence on the body's inflammatory response and immune function of patients.
