摘要(Abstract)：

目的:研究老年慢性阻塞性肺疾病(COPD)患者体质指数(BMI)与肺功能的关系。方法:270例老年COPD稳定期患者作为研究组,同期90例老年健康者为对照组,比较两组被检者性别、年龄、BMI和心率等一般临床资料以及空腹血糖和甘油三酯(TG)、总胆固醇(TCHO)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)等部分生化指标;按照BMI将研究组患者分为A组(低BMI)、B组(正常BMI)和C组(超BMI),比较3组COPD患者深吸气量(IC)、第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、一秒率(FEV1/FVC)和FEV1实测值与预计值之比(FEV1/预测值)等部分肺功能指标。结果:研究组心率高于对照组,BMI、TCHO、HDL-C和LDL-C水平低于对照组,差异具有统计学意义(P<0.05);研究组A组患者IC、FEV1、FVC、FEV1/FVC和FEV1/预测值均明显低于B组,C组患者IC、FEV1、FVC、FEV1/FVC和FEV1/预测值明显高于B组,差异具有统计学意义(P<0.05)。结论:老年COPD患者的BMI值越高,其肺功能越好。
Objective: To investigate the correlation between body mass index and pulmonary function of elderly patients with chronic obstructive pulmonary disease( COPD). Methods: A total of 270 elderly patients with stable COPD were selected as the study group,while 90 cases of elderly healthy subjects were selected as the control group. Comparing general clinical data of gender difference,age,BMI and heart rate,as well as biochemical indicators in the two groups of patients,such as TG,TCHO,HDL-C,LDL-C. According to the BMI division standard,270 patients in the study group were divided into three groups: A-low BMI,B-normal BMI,C-super BMI group according to the BMI division index. Comparing the indexes of pulmonary function in three groups of COPD patients with different BMI: IC,FEV1,FVC,FEV1/FVC,FEV1/predicted value. Results: Heart rate of study group were higher than control group. The levels of BMI,TCHO,HDL-C and LDL-C in the study group were lower than those in the control group,differences were statistically significant( P < 0. 05). The IC,FEV1,FVC,FEV1/FVC and FEV1/prediction value in group A were significantly lower than that of group B. The IC,FEV1,FVC,FEV1/FVC and FEV1/prediction value in group A were significantly higher than that of group B,differences were statistically significant( P < 0. 05). Conclusion: The higher the BMI value in elderly patients with COPD,the better the pulmonary function.
