新生儿支原体肺炎血清心肌酶和肌钙蛋白I水平检测Detection of Myocardial Enzyme and Troponin I and Its Clinical Significance in Mycoplasma pneumoniae Pneumonia of Neonates

摘要(Abstract):
目的:观察支原体肺炎与细菌性肺炎新生儿心肌酶及肌钙蛋白I(cTnI)的水平。方法:临床确诊为新生儿支原体肺炎(MPP组)55例和新生儿细菌性肺炎(N-MPP组)60例,于入院后第2天、第7天和2周测定血清心肌酶及cTnI,并与56例健康新生儿(对照组)进行比较。结果:MPP组、N-MPP组患儿入院第2天、第7天心肌酶及cTnI均高于对照组(P<0.01),入院后第2天N-MPP组高于MPP组(P<0.05或P<0.01),第7天MPP组高于N-MPP组(P<0.05或P<0.01);入院后2周N-MPP组心肌酶及cTnI基本降至正常,与对照组比较,差异无统计学意义(P>0.05),而MPP组仍明显高于对照组(P<0.01)。结论:新生儿支原体肺炎较细菌性肺炎心肌酶及cTnI水平升高延迟出现且恢复慢,不同时间检测心肌酶及cTnI,可了解患儿心肌损害情况,及时给予保护心肌治疗。
Objective: To observe the expression levels of myocardial enzymatic and troponin I in Mycoplasma pneumoniae pneumonia and bacterial pneumonia in neonates,and to explore its effects on myocardial damage.Methods: Fifty-five cases of confirmed neonatal M.pneumoniae pneumonia(group MP) and 60 cases of confirmed neonatal bacterial pneumonia(group N-MP) were collected.The expressions of myocardial enzymes(ME) and troponin I(cTnI) of all cases were detected on the 2nd day,7thday,and in 2 weeks after admission,and compared with those of 56 normal newborns(control group).Results: The expression levels of ME and cTnI in group MP and group N-MP were higher than those in control group on the 2ndday and 7thday after admission(P < 0.01),and the levels of ME and cTnI in group N-MP were higher than those in group MP on the 2ndday after admission(P < 0.05or P < 0.01),but were lower on the 7thday after admission(P < 0.05 or P < 0.01).Two weeks later after admission,the levels of ME and cTnI in group N-MP returned to normal,and were close to those of control group,(P > 0.05).But the levels of ME and cTnI in group MP kept significantly higher than those in group N-MP and control group(P < 0.01).Conclusion: Comparing with group N-MP,the expressions of ME and cTnI in group MP appear later and restore slower.ME and cTnI should be detected in different times and sowe can closely observe the situation of myocardial damage,and give in time treatment for myocardium protection,and to avoid delay.
