1214名某社区人群代谢综合征相关指标及其与血清25-羟维生素D水平的相关分析Correlation between Metabolic Syndrome and Serum 25-Hydroxyvitamin D in Some Community Population of Guiyang

摘要(Abstract):
目的:探讨某社区人群代谢综合征相关指标及其与血清25-羟维生素D[25-(0H) D]水平的相关性。方法:收集2009年贵阳市居民健康横断面调查的1 214份调查资料(资料齐全,男516名、女698名,年龄20~79岁),按代谢综合征诊断标准分为代谢综合征组及非代谢综合征组、再根据血清25-(0H) D水平分为25-(0H)D正常组及异常组[血清25-(0H) D降低];比较各组性别比、年龄、腰围(WC)、体质指数(BMI)、血钙(Ca)、血磷(P)、空腹血糖(FPG)、餐后2 h血糖(PPG)、收缩压(SBP)、舒张压(DBP)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、总胆固醇(TC)、尿酸(UA)、肌酐(Cr)及25-(0H) D;采用Logistic回归分析不同性别25-(0H) D下降的危险因素。结果:代谢综合征组的年龄、WC、BMI、FPG、PPG、SBP、DBP、TG、HDL-C、TC及UA均显著高于无代谢综合征组,血P显著低于无代谢综合征组(P <0. 01); 25-(0H) D水平等其他指标比较,差异无统计学意义(P> 0. 05);血清25-(0H) D正常组男性与异常组男性进行代谢综合征各组分比较,差异无统计学意义(P> 0. 05); 25-(0H) D异常组女性的WC及PPG血糖均显著高于血清25-(0H) D正常组女性(P=0. 007、0. 000 8); Logitic回归分析显示WC是人群25-(0H) D下降危险因素(OR=0. 820,P=0. 004)。结论:25-(0H) D水平缺乏减少女性人群的WC及PPG显著高于25-(0H) D水平正常女性人群,WC是调查人群25-(0H) D缺乏的独立危险因素。
Objective: The purpose of this study was to investigate the correlation between metabolic syndrome factors such as obesity,blood glucose,blood lipid,uric acid,blood pressure and so on and serum 25-hydroxyvitamin D [25-( OH) D] in a community. Methods: A total of 1 214 survey data( including 516 males and 698 females aged 20-79) from the 2009 health cross-sectional survey of Guiyang residents were collected. According to the diagnostic criteria of metabolic syndrome,the data was divided into the metabolic syndrome group and the non-metabolic syndrome group. According to the serum 25-( OH) D level,the data was divided into the 25-( OH) D normal group and the abnormal group [serum 25-( OH) D decreased]. The sex ratio of each group,age,waist circumference( WC),body mass index( BMI),blood blood calcium( Ca),phosphorus( P),fasting plasma glucose( FPG),2 h postprandial blood sugar( PPG),systolic blood pressure( SBP),diastolic blood pressure( DBP),triglyceride( TG),low density lipoprotein cholesterol( LDL-C),high density lipoprotein cholesterol( HDL-C),total cholesterol( TC) and uric acid( UA),creatinine( Cr) and 25( 0 h)D; Logistic regression was used to analyze the risk factors for 25-( 0 H) D decline in different genders.Results: The age,WC,BMI,FPG,PPG,SBP,DBP,TG,hdl-c,TC and UA of the metabolic syndrome group were significantly higher than those of the non-metabolic syndrome group,and the blood P was significantly lower than that of the non-metabolic syndrome group( P < 0. 01). Comparison of other indicators such as 25-( OH) D level showed no statistically significant difference( P > 0. 05). There was no significant difference in the components of metabolic syndrome of men between the normal group and the abnormal group( P > 0. 05). The WC and PPG blood glucose levels of women in the 25-( OH)D group were significantly higher than those in the 25-( OH) D group( P = 0. 007,0. 000 8). Logistic regression showed that WC was a risk factor for 25-( OH) D decline( OR = 0. 820,P = 0. 004).Conclusion: The lack of 25-( OH) D level reduces WC of female population and PPG is significantly higher than that of female population with normal 25-( OH) D level; WC was an independent risk factor for lack of 25-( OH) D in men and women.
