肺炎支原体快速培养及血清抗体检测在儿童支原体肺炎早期诊断中的价值

摘要(Abstract):

目的:探讨肺炎支原体(MP)的快速培养及血清抗体检测在儿童支原体肺炎早期诊断中的价值。方法:选取儿科收治疑似MP感染的100名儿童作为实验组,并将同期在儿科门诊进行健康体检的60名健康儿童作为正常对照组;对实验组(入院首日未进行抗生素治疗前)和对照组同时取两侧腭弓及咽腔分泌物进行MP快速鉴定培养、药物敏感试验,酶联免疫吸附试验(ELISA),检测血清肺炎支原体IgM抗体(MP-IgM);实验组在接受抗生素治疗两周后再次进行MP快速鉴定培养、ELISA检测血清MP-IgM,对检测结果进行分析。结果:实验组在接受治疗前MP培养及血清MP-IgM阳性率均明显高于正常对照组阳性率,差异有统计学意义(P<0.05);实验组在接受抗生素治疗两周后,MP培养阳性率下降,而血清MP-IgM阳性率上升,差异有统计学意义(P<0.05);在药敏试验中,MP对阿奇霉素及喹诺酮类抗生素有较高的敏感度,对红霉素、克林霉素耐药率较高。结论:肺炎支原体快速鉴定培养法在儿童支原体肺炎发病初期阳性率优于血清MP-IgM检测,联合检测并结合药物敏感试验,为临床提供重要的早期诊断与治疗依据。
Objective: To investigate the value of rapid cultivation of Mycoplasma pneumoniae( MP)and detection of serum antibody in early diagnosis of mycoplasma pneumonia in children. Methods: A total of 100 children with suspected MP infection in the pediatric department were selected as the experimental group,and 60 healthy children accepting physical examination in the outpatient department of pediatrics in the same period were selected as the normal control group. For study group( before the treatment of antibiotics) and the control group,rapid identification culture and drug sensitivity test of MP were carried out,and the enzyme linked immunosorbent assay( ELISA) was used to detect MP antibody( MP-IgM) at the same time. After two weeks of antibiotics treatment,the rapid identification culture of MP was performed and the serum MP-IgM was detected by ELISA again in the study group.Analyzing test results. Results: Before the treatment of antibiotics,the positive rate of MP culture and serum MP-IgM in the study group was significantly higher than that in the normal control group before receiving the treatment( P < 0. 05). After two weeks of antibiotics treatment,the positive rate of MP culture in the study group was decreased and the serum MP-IgM positive rate increased( P < 0. 05).For the drug sensitivity test,MP had a high sensitivity to azithromycin and quinolone antibiotics,and had high drug resistance rate for erythromycin and clindamycin. Conclusions: MP positive rate detected by rapid identification culture is higher than detection of serum MP-IgM in early stage of mycoplasma pneumonia in children. Combined detection plus drug sensitivity test provide the important basis for early diagnosis and treatment of mycoplasma pneumonia.
