心力衰竭患者血浆利钠肽水平及萘哌地尔对其的影响

摘要(Abstract):

目的 :观察心力衰竭患者血浆心钠素 (ANP)、脑钠素 (BNP)及C 型利钠肽 (CNP)浓度与心力衰竭严重程度、左室功能的关系及萘哌地尔对其的影响。方法 :用放免法检测 4 7例心力衰竭患者 (CHF组 )及 18例健康体检者 (对照组 )血浆ANP、BNP和CNP浓度。然后将CHF组随机分为常规治疗组 2 4例和加用萘哌地尔组 2 3例 ,1月后重复上述检查。结果 :CHF组血浆ANP、BNP浓度显著高于对照组 (P <0 0 5 ) ,均随NYHA分级程度的加重而显著增加 ,且均与左室射血分数呈明显负相关。轻度心衰患者血浆CNP浓度显著高于对照组 (P <0 0 5 ) ,而中、重度心衰无明显改变。CHF组治疗后血浆ANP和BNP浓度均显著下降 (P <0 0 5 ) ,但以萘哌地尔治疗组下降更为显著 (P <0 0 5 )。结论 :利钠肽与CHF的发生发展有关 ,在常规治疗的基础上加用萘哌地尔治疗可进一步降低心力衰竭患者ANP和BNP水平。
Objective: To explore the relations between the plasma concentrations of atrial natriuretic peptide(ANP),brain natriuretic peptide(BNP)and C type natriuretic peptide(CNP)and the severity of heart failure and the left ventricular function in patients with congestive heart failure(CHF) and the effect of naftopidil on them. Method: Plasma ANP、BNP and CNP concentrations of 47 patients with congestive heart failure (CHF) and 18 healthy subjects (control group, CG) were measured with radioimmunoassay. The CHF patients were then divided into 2 groups randomly. Patients in group 1 received naftopidil in addition to routine therapy ( group NPD, n =23) while patients in group 2 received routine therapy only (group RT, n =24) for 1 month. Result: 1. The plasma ANP and BNP concentrations of patients were higher than those of normal controls ( P <0 05).They also increased in accordance with the severity of heart failure classified by NYHA’s classification and had a significant negative correlation with the left ventricular ejection fraction. 2. Compared with the normal controls, the plasma CNP concentration significantly increased in patients with mild CHF, while no significant change of that was found in patients with moderate and serious CHF. 3. Both of the therapies were effective in decreasing the levels of ANP and BNP ( P <0 05), and the decrease in group NPD was more obvious ( P <0 05). Conclusions: ANP、BNP and CNP could play an important role in the pathophysiological changes of CHF. Naftopidil in addition to the routine therapy can decrease the levels of ANP and BNP more efficiently [
