HBsAg阳性产妇血清HBV-DNA水平与新生儿HBV感染的关系

摘要(Abstract):

目的:探讨乙型肝炎病毒表面抗原(HBs Ag)阳性产妇血清乙型肝炎病毒脱氧核糖核酸(HBV-DNA)水平与新生儿乙型肝炎病毒(HBV)感染的关系。方法:采集67例HBs Ag阳性产妇分娩前的外周静脉血及其分娩的67例新生儿脐血,用时间分辨免疫荧光法检测乙肝5项指标,实时荧光定量PCR法检测产妇血清HBVDNA含量,记录e抗原阳性组和e抗原阴性组HBs Ag阳性的产妇例数及脐血阳性的新生儿数,计算HBs Ag阳性产妇的母婴HBV传播率;记录HBV-DNA低水平组(拷贝数<106IU/L)、HBV-DNA中水平组(拷贝数为106~108IU/L)和HBV-DNA高水平组(拷贝数≥109IU/L)中HBs Ag阳性的产妇例数和脐血阳性的新生儿数,比较HBV-DNA高、中、低水平组中的母婴HBV传播率。结果:67例HBs Ag阳性产妇的新生儿脐血中HBs Ag阳性24例,母婴传播率为35.82%(24/67),e抗原阳性组HBs Ag阳性产妇的母婴HBV传播率(54.29%)高于e抗原阴性组产妇(15.639%),差异有统计学意义(P<0.01,χ2=10.87);不同水平HBV-DNA拷贝数HBs Ag阳性产妇的母婴HBV传播率从高到低依次为高水平组、中水平组及低水平组,两两比较差异有统计学意义(P<0.05)。结论:产妇感染HBV可能引起母婴垂直传播,e抗原阳性及血清HBV-DNA含量高的产妇母婴HBV传播率高。
Objective: To investigate the relationship between Hepatitis B virus surface antigen positive mothers' s HBV-DNA content level and HBV infection in Newborn. Methods: Maternal venous blood was collected 24 hours before delivery,and neonatal umbilical cord blood was collected under sterile condition at the time of delivery. Then,immunofluorescence method was adopted to detect five indexes of hepatitis B virus by time differences; real-time fluorescence quantitative PCR method was used to detect the content of serum HBV-DNA. Recording number of puerpera and umbilical cord blood newborn of HBe Ag + group and HBe Ag- group,calculating maternal infectious rate of HBV in HBe Ag + mothers; recording number of puerpera and umbilical cord blood newborn of HBe Ag + group in HBV-DNA low-level group,HBV-DNA medium level group and HBV-DNA high level group in or-der to compare maternal HBV transmission rate in three groups. Results: 67 HBsA g positive mothers,24 cases of HBsA g positive in newborns,the rate of maternal transmission is 35. 82%. HBsA g positive puerpera transmission rate( 54. 29%) is higher than HBsA g negative puerpera( 15. 639%),difference is statistically significant( P < 0. 01,χ2= 10. 87); HBsA g positive puerpera transmission rate in various levels of HBV-DNA copy numbers from high to low is high level group,medium level group and low level group,comparison of any two groups are statistically significant( P < 0. 05). Conclusions: Maternal HBV infection can cause maternal vertical transmission,HBsA g positive and high serum HBVDNA content maternal HBV transmission rate is high.
