燃煤型砷中毒患者血清总蛋白、白蛋白和胆碱酯酶的变化
A Study on the Changes of Serum Contents of TP,ALB,and CHE in Coal-burning Caused Arsenism Patients and Its Diagnostic Significance

摘要(Abstract):

目的:了解燃煤型砷中毒患者血清中总蛋白(TP)、白蛋白(ALB)和胆碱酯酶(CHE)的变化,探讨其在砷中毒患者肝损伤诊断中的意义。方法:采用生物化学法检测燃煤型砷中毒患者血清中TP,ALB含量和CHE活性。结果:(1)砷中毒患者血清中TP,ALB含量及CHE活性在肝脏B超正常与异常组均明显低于正常对照组(P<0.01)。肝脏B超异常组患者血清中CHE活性较肝脏B超正常组明显降低(P<0.05),而TP和ALB含量两组间差异无统计学意义(P>0.05);(2)患者肝脏B超异常率为47.54%(29/61);血清中TP,ALB含量及CHE活性异常率分别为77.05%,65.57%和96.72%,均明显高于肝脏B超异常率,差异有统计学意义(P<0.05);肝脏B超正常与异常组患者血清中CHE活性异常率均明显高于TP,ALB的异常率(P<0.05或P<0.01)。结论:砷可影响人体肝脏合成功能;患者血清中TP,ALB含量及CHE活性较肝脏B超检查砷致人体肝损伤情况更为敏感;CHE活性较TP,ALB含量能更好的反映砷中毒患者肝脏损伤情况;动态监测三指标的改变对砷中毒患者肝损伤的早期发现、早期诊断及早期防治有一定的参考价值。
Objective: To observe the content variation of total protein(TP),Albumin(ALB),and the activity of cholinesterase(CHE) in patients' serum,and to explore its significance in diagnosis of arsenism liver damage.Methods: Biochemical methods were used to detect the contents of TP,ALB and the activity of CHE in the serum of arsenism patients.Results: 1.The contents of TP,ALB and the activity of CHE in arsenism patients were obviously lower than those in normal contrasting group,no matter the liver B ultrasonography results were normal(patient group 1,P1) or abnormal(patient group 2,P2)(P<0.01).The activity of serum CHE in group P1 was obviously lower than that in group P2 (P<0.05).But there was no statistical difference of serum TP and ALB between the two groups(P>0.05).2.In both patient groups,the total abnormal rates of serum contents of TP,ALB and CHE activity were 77.05%,65.57% and 96.72% respectively,which were obviously higher than the abnormal rate of liver B ultrasonography examination(29/61,47.54%),and the abnormal rate of CHE activity was higher than those of TP and ALB contents(P<0.05 or P<0.01).Conclusions: Arsenic can influence the human liver synthetical function.The serum TP,ALB and,specially,CHE are more sensitive in reflecting arsenic-induced liver damage than B ultrasonography examination,which are valuable in early detection,diagnosis,prevention and control of liver damage in arsenism patients.So,it is important to dynamically monitor the three indexes of patients.
