摘要(Abstract)：

目的:探讨醋酸曲普瑞林联合米非司酮治疗腹腔镜术后子宫内膜异位症(EM)的疗效。方法:128例腹腔镜术后EM患者随机分为观察组和对照组(各64例),对照组EM给予醋酸曲普瑞林治疗,观察组在对照组治疗基础上加用米非司酮,比较两组EM患者治疗前及治疗结束时血清基质金属蛋白酶-9(MMP-9)和碱性成纤维细胞生长因子(b FGF)水平,比较两组EM患者治疗效果及治疗不良反应,同时比较治疗后6个月、12个月、18个月及24个月时两组EM患者累计妊娠率及EM复发率。结果:观察组总有效率为96.87%,对照组总有效率为81.25%,差异有统计学意义(P<0.05);两组患者治疗结束时血清MMP-9和b FGF水平均低于治疗前(P<0.05),且观察组治疗后血清MMP-9和b FGF水平显著低于对照组(P<0.05);观察组治疗后6个月、12个月、18个月及24个月时累计妊娠率高于对照组(P<0.05),EM复发率显著低于对照组(P<0.05)。结论:EM患者腹腔镜术后联合醋酸曲普瑞林与米非司酮治疗能有效提高疗效,降低患者术后复发率,改善患者预后,值得临床应用。
Objective: To explore curative effect of triptorelin acetate combined with mifepristone in treatment of patients with endometriosis after laparoscopic surgery. Methods: A total of 128 cases of patients with endometriosis( EM) who underwent with laparoscopic surgery were divided into control group( n = 64) and observation group( n = 64) from June 2011 to June 2012. Control group received triptorelin acetate while observation group received triptorelin acetate combined with mifepristone. The levels of MMP-9 and b FGF of two groups were compared before and after treatment. The curative effect and untoward effect of two groups were compared. The EM relapse rate and cumulative pregnancy rate of two groups were compared 6 month after treatment,12 month after treatment,18 month after treatment and 24 month after treatment. Results: The total effective rate of control group and observation group were 81. 25% and 96. 87%,and the difference was statistically significant( P < 0. 05). The levels of MMP-9 and b FGF after treatment in two groups were significantly lower than before treatment( P < 0. 05),and the levels of MMP-9 and b FGF of observation group were significantly lower than their counterparts of control group( P < 0. 05). The cumulative pregnancy rate of observation group were higher than that of control groups 6 month after treatment,12 month after treatment,18 month after treatment and 24 month after treatment( P < 0. 05),while the cumulative recurrence rate of observation group were significantly lower than that of control group( P < 0. 05). Conclusion: Triptorelin acetate combined with mifepristone can effectively improve the curative effect of EM after laparoscopic surgery by decreasing the serum levels of MMP-9 and b FGF,reduce the postoperative recurrence rate of EM,improve prognosis of patients with EM.
