血清MUC1和Hpa检测对甲状腺乳头状癌及淋巴转移的诊断价值

摘要(Abstract):

目的:检测甲状腺乳头状癌患者血清黏蛋白1(MUC1)和乙酰肝素酶(Hpa)水平,探讨两者在甲状腺乳头状癌及淋巴转移诊断中的价值。方法:选取甲状腺乳头状癌患者60例、状腺良性结节患者80例以及健康体检者60例为研究对象,检测3组被检者血清中MUC1和Hpa水平,同时对比甲状腺乳头状癌淋巴结转移与无淋巴结转移患者血清MUC1和Hpa水平;根据所有被检者血清MUC1和Hpa检测结果绘制ROC曲线,分析MUC1和Hpa两项指标对甲状腺乳头状癌的诊断价值。结果:甲状腺乳头状癌组患者血清中MUC1和Hpa水平明显高于甲状腺良性结节组和健康对照组,差异具有统计学意义(P<0.05);有淋巴结转移的甲状腺乳头状癌患者血清中MUC1水平显著高于无淋巴结转移患者,差异有统计学意义(P<0.05),而血清Hpa水平与是否有淋巴结转移无关(P>0.05);绘制ROC曲线比较显示,MUC1对鉴别甲状腺乳头状癌的价值显著高于Hpa检测,差异有统计学意义(P<0.05)。结论:血清MUC1与Hpa水平对甲状腺乳头状癌的诊断均具有一定的临床意义。
Objective: To evaluate the levels of Mucins1( MUC1) and Heparanase( Hpa) as marker in diagnosis of papillary thyroid neoplasms and lymph node metastasis. Methods: Sixty patients with papillary thyroid neoplasms,80 patients with benign thyroid nodules and 60 normal persons were enrolled in this study. The levels of serum MUC1 and Hpa of three groups were measured and the differences were contrasted. The levels of serum MUC1 and Hpa of patients with and without lymph node metastasis were compared. The receiver operating characteristic curve( ROC curve) was drawn according to the levels of MUC1 and Hpa in all subjects and diagnostic value of serum MUC1,Hpa in papillary thyroid neoplasms were analyzed. Results: The levels of MUC1 and Hpa in papillary thyroid neoplasms group were significantly higher than those in other two groups( P < 0. 05). The expression of MUC1 in patients with lymph node metastasis was significantly higher than patients without lymph node metastasis( P < 0. 05),but the level of Hpa in serum was not significantly different between patients with and without lymph node metastasis( P > 0. 05). The MUC1 showed significantly higher clinical value in diagnosis of thyroid cancer than Hpa according to ROC curve analysis( P < 0. 05). Conclusion: The levels of MUC1 and Hpa both have important value in the diagnosis of papillary thyroid neoplasms. The serum MUC1 may have better application value than Hpa,and MUC1 as an important biological detection index is closely related with thyroid cancer incidence,predicting lymph node metastasis and prognosis.
