中性粒细胞-淋巴细胞比值与急性脑梗死的相关性研究

摘要(Abstract):

目的:研究外周血中性粒细胞-淋巴细胞比值(NLR)与急性脑梗死发生的相关性。方法:收集78例急性脑梗死患者和同期63例健康对照者一般资料,记录白细胞计数、中性粒细胞计数、淋巴细胞绝对值和NLR检测结果,进行统计学分析;对两组被检者资料比较具有统计学意义的因素进行多因素Logistic回归分析,并绘制ROC曲线。结果:两组被检者的高血压史、糖尿病史、白细胞总数、中性粒细胞计数、淋巴细胞计数及NLR指标比较,差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示,NLR是急性脑梗死发生的独立危险因素(P<0.05),ROC曲线中白细胞计数、中性粒细胞计数、淋巴细胞绝对值、NLR对急性脑梗死的曲线下面积分别为0.764、0.856、0.298和0.869,其中NLR预测发生急性脑梗死的最佳诊断值为2.66(特异性为90.5%,敏感性为74.4%,P<0.001)。结论:外周血NLR对发生急性脑梗死具有一定的预测价值。
Objective: To study the correlation between neutrophil to lymphocyte ratio( NLR) and acute cerebral infarction. Methods: 78 patients with acute cerebral infarction and 63 healthy controls were collected and received routine laboratory hematology tests including white blood count,neutrophil,lymphocyte and NLR. The clinical outcomes at discharge were retrospectively reviewed and analyzed. Factors with statistical significance were analyzed by logistic regression and the ROC curve was plotted. Results: The incidence of hypertension and diabetes,number of WBC neutrophils and lymphocytes and NLR of both groups were significantly different( P < 0. 05). Multi factors Logistic regression analysis showed that NLR was an independent risk factor for acute cerebral infarction. The areas under the curve of WBC,absolute neutrophils and lymphocytes,NLR for predicting acute cerebral infarction were 0. 764,0. 856,0. 298 and 0. 869 respectively. The optimal cut-off value of NLR was 2.66,with a specificity of 9. 5% and a sensitivity of 74. 4% for predicting acute cerebral infarction.Conclusion: Early peripheral NLR plays a role in predicting acute cerebral infarction.
