喉癌组织中KAI1和E-cad表达及相关性
Expression of KAI1 and E-Cadherin in Laryngocarcinoma Tissues and Their Relationship

摘要(Abstract):

目的:观察人喉癌中肿瘤转移抑制基因KAI1和钙黏附蛋白E-cadherin(E-cad)的表达。方法:40例喉癌标本和20例正常咽部黏膜组织,用免疫组化法检测KAI1和E-cad蛋白的表达情况。结果:(1)在喉癌组织中,KAI1蛋白表达的阳性率为62.5%(25/40),E-cad蛋白阳性率为47.5%(19/40),均明显低于对照组,差异有统计学意义(P<0.05);(2)两种蛋白表达与肿瘤临床分期及淋巴结转移密切相关,差异有统计学意义(P<0.05);(3)在同一标本中,二者表达有很好的相关性和一致性,Pearson列联系数(P)=0.529 0,一致性系数Kap-pa=0.739 3。结论:(1)KAI1和E-cad蛋白的表达与喉癌的进展和转移密切相关,检测其表达异常对判断肿瘤的分化,临床进展以及推测预后有一定参考价值。(2)KAI1和E-cad蛋白在喉癌组织中的表达具有很好的相关性和一致性,推测二者在喉癌的侵袭和转移中可能相互作用形成复合体,存在着正反馈关系,二者的低表达或失活使细胞间黏附减弱或丧失,肿瘤细胞易于脱离原发灶而发生转移。
Objective: To observe the expression of tumor metastasis suppressor gene KAI1 and E-cadherin(E-cad) in laryngocarcinoma.Methods: Immunohistochemistry SP method was used to examine the expression levels of KAI1 and E-cad in 40 specimens of laryngocarcinoma and 20 specimens of healthy people.Results:(1) In laryngocarcinoma tissues,positive expression rate of KAI1 protein was 62.5%(25/40) and that of E-cad protein was 47.5%(19/40),and both were lower than those in control group(P<0.05).(2) The expression of the two proteins was closely correlated with tumor clinical staging and lymph node metastasis.(3)In the same sample,the expression of KAI1 and E-cad showed close relationship and good consistency.Pearson contingency coefficient was 0.5290 and consistency coefficient Kappa was 0.7393.Conclusions:(1)The expression of KAI1 and E-cad protein closely related with development and metastasis of laryngocarcinoma.Detection of their expression has some reference value in judging tumor differentiation and clinical progress,and predicting prognosis.(2) The expression of KAI1 shows close relationship and good consistency with that of E-cad,suggesting that they may form a complex in invasion and metastasis of laryngocarcinoma.Low expression and inactivation of them lead to attenuation or loss of cell-cell adhesion.In this way,tumor cell can more easily detach from primary focus and to metastasize.
