2型糖尿病合并原发性高血压患者肥胖与血压晨峰相关性研究

摘要(Abstract):

目的:了解2型糖尿病(T2DM)合并原发性高血压患者(EH)肥胖与血压晨峰(MBPS)的相关性。方法:采用横断面研究的方法,选取96例T2DM合并EH患者(男47例,女49例),分为男女2组,分别记录2组患者年龄、身高、体重、腰围,计算体重指数(BMI)、腰身比(WHR),同时24 h动态血压监测,计算MBPS;使用Pearson相关分析和多元线性逐步回归方法,分析男女组患者年龄、身高、体重、腰围、BMI及WHR各指标与MBPS的相关性。结果:在男女2组患者中,腰围、WHR、身高、体重、年龄、BMI与MBPS的发生无明显相关性(P>0.05),性别与MBPS亦无相关性(r=0.147,P=0.154);以MBPS为因变量的多元线性逐步回归分析结果显示男女2组中,患者BMI与MBPS水平呈正相关(P<0.05),而身高、体重、年龄与MBPS无明显相关性(P>0.05),患者WHR、腰围与MBPS的发生仅在男性组中呈正相关(P<0.05)。结论:T2DM合并EH患者的BMI均与MBPS水平呈正相关,男性腰围及WHR与MBPS水平呈正相关。
Objective: To explore the correlation between obesity and morning blood pressure surge( MBPS) in type 2 diabetes mellitus( T2DM) patients with essential hypertension( EH). Methods:All total 96 subjects( 47 male patients and 49 female patients) were selected from the department of endocrinology and metabolism in affiliated hospital of Guizhou medical university from June 2014 to August 2014,who were divided into two groups according to gender. The age,height,weight,waistline were recorded,thus BMI and WHR were calculated. Meanwhile,MBPS were examined by 24 h ambulatory blood pressure monitoring. The Pearson correlation analysis and multiple stepwise regression analysis were adopted to analyze the correlation between all parameters and MBPS. Results: In two groups,there existed no obvious correlation between the age,height,weight,waistline,BMI,WHR and MBPS( P > 0. 05). There was also no correlation between gender and MBPS( r = 0. 147,P =0. 154). In the multiple linear stepwise regression analysis,BMI in male and female were associated significantly in a positive manner with MBPS( P < 0. 05) while height,weight and age were not obviously associated with MBPS( P > 0. 05). The positive correlation between WHR,waistline and MBPS only occurred in male patients( P < 0. 05). Conclusion: In T2 DM patients with essential hypertension( EH),BMI in male and female are associated significantly in a positive manner with MBPS. The levels of waistline and WHR in male are associated significantly in a positive manner with MBPS.
