口腔锥形束CT在测量青少年上颌骨发育不良中的价值

摘要(Abstract):

目的:探讨口腔锥形束CT(CBCT)在测量青少年上颌骨三维形态中的价值。方法:40例青少年,根据CBCT的SNA(由蝶鞍中心、鼻根点及上牙槽座点所构成的角)与SNB(由蝶鞍中心、鼻根点及下牙槽座点所构成的角)大小不同均分为上颌骨正常组(正常组)和上颌骨发育不足组(发育不足组),采用CBCT建立上颌骨三维数据,测量上牙槽座点(A)、鼻骨最前点(ANB)、前鼻棘点(ANS)、后鼻棘点(PNS)、颅底点(Ba)、蝶鞍点(S)、眶下点(Or)、耳点(P)、上颌尖牙牙槽缘点(SPC)及上颌第一磨牙缘点(SPM)到矢状面(S)、水平面(H)及冠状面(C)的距离,比较这些数据在两组被检测者男女性之间的差异。结果:按男、女性别分别比较,发育不足组中A点到HP、CP距离,ANB点到CP距离,ANS到HP、CP距离,PNS点到CP的距离,SPM(L)点到HP、CP、SP的距离,SPM(R)点到HP的距离,SPC(L)点到HP、CP、SP的距离,SPC(R)点到HP、CP、SP的距离均小于正常组(P<0.05)。结论:CBCT建立的三维数据可用于诊断青少年上颌骨发育不足。
Objective: To investigate the value of oral cone beam CT( CBCT) in the measurement of the three-dimensional shape of adolescents' maxillary. Methods: According to difference of CBCT SNA( composed of sella center,nasal root and point a angle) and SNB( composed of sella nasion center,and point B angle),40 cases of teenagers were divided into maxillary normal group and no maxillary hypoplasia group. CBCT was adopted to establish three-dimensional data of maxillary,and measure the distance from the alveolar point( A),the point of nasal bone( ANB),nasospinale( ANS),staphylion( PNS),skull base point( Ba),sella point( S),infraorbital point( Or),ear point( P),maxillary canine alveolar edge point( SPC) and the edge point of the maxillary first molar( SPM) to the sagittal plane( SP),to horizontal plane( HP) and to coronal plane( CP),respectively. The differences of these data detected between the sexes in the two groups were compared. Results: The distance from A to HP and CP,the distance from ANB to CP,the distance from ANS to HP and CP,the distance fromPNS to CP,the distance from SPM( L) to HP,CP and SP,the distance from SPM( R) to HP,the distance from SPC( L) to HP,CP and SP,and the distance from SPC( R) to HP,CP and SP of maxillary hypoplasia group were all less than their counterparts of maxillary normal group. Conclusion:The three-dimensional data established by CBCT can be used for the diagnosis of maxillary hypoplasia in adolescents.
