二孩孕妇Rh血型抗体与新生儿溶血病的关系

摘要(Abstract):

目的:探讨二孩孕妇Rh血型抗体与新生儿溶血病(HDN)的关系。方法:选取180例产检的二孩孕妇作为研究组,同期首次妊娠的180例孕妇作对照组,于分娩前鉴定2组孕妇的Rh血型、不规则抗体及其特异性,收集2组新生儿血液检测Rh抗原血型(D、d、E、e、C及c抗原)、记录HDN发生情况;比较2组孕妇不规则抗体筛查阳性率、2组孕妇中RhD抗原阴性孕妇的不规则抗体阳性率,比较2组新生儿Rh-HDN的发生率,分析二孩孕妇Rh血型抗体与HDN的关系。结果:研究组检测出6例孕妇不规则抗体阳性(3.3%)、分别为抗E(66.6%)、抗D(16.7%)及抗C(16.7%),对照组孕妇未检测出不规则抗体(0.0%),差异有统计学意义(P<0.05);研究组的新生儿中5例发生Rh-HDN(2.8%),对照组的新生儿未发生Rh-HDN,差异有统计学意义(P<0.05);发生Rh-HDN的患儿不规则抗体效价均≥1∶32,直接抗体2+以上。结论:应重视对二孩孕妇多种Rh血型抗原的检测。
Objective: To explore the relationship between Rh blood group antibody and hemolytic disease of newborn in two-child pregnant women. Methods: 180 two-child pregnant women with were selected as the study group and 180 pregnant women for the first time in the same period as the control group. The Rh blood group, irregular antibody and their specificity of the two groups of pregnant women were identified before delivery, and the blood group of Rh antigen(D, d, E, e, C and c antigen) was detected in the blood of the two groups of newborns. The occurrence of HDN was recorded. The positive rate of irregular antibody screening in two groups of pregnant women was compared, and the positive rate of irregular antibody in pregnant women with negative RhD antigen in the two groups was compared. The incidence of Rh-HDN in two groups was compared, and the relationship between Rh blood group antibody and HDN in two-child pregnant women was analyzed. Results: In the study group, 6 pregnant women were positive for irregular antibodies(3.3%), which were respectively anti-E(66.6%), anti-D(16.7%) and anti-C(16.7%). No irregular antibody(0.0%) was detected in the control group, and the difference was statistically significant(P<0.05). The titer of irregular antibody in children with Rh-HDN was ≥1 ∶32, and the direct antibody was more than 2+. Conclusion: Attention should be paid to the detection of various Rh blood group antigens in second-child pregnant women.
