时域及频域分析检测心室晚电位临床应用研究A Study of the Clinical Application of Time-Domain and Frequency-Domain Analyses for Detecting Ventricular Late Potential

摘要(Abstract):
报道应用时域及频域分析方法对119例正常人,35例心肌梗塞及11例室性心动过速病人进行检测心室晚电位(VLP)的结果,提出:运用国产仪器的VLP阳性标准;时域分析受截止频率的影响;滤波后总QRS时限(QRSt)及各频段面积在性别间均有显著性差异(P<0.05~0.001);频域分析以叠加200次心动周期为好,能获得满意的信/噪比;从QRS的起点向后60ms为开始点向后延140ms作为心电分析段(A60)为较好分析段。
The results of detecting ventricular late potential (VLP) with time-domain (TD) and frequency-domain (FD) analyses of signal-averaged electrocardi- ography (SAECG) in H9 normal subjects (N), 35 patients with [myocardial infarction (MI) and 11 patients with ventricular tachycardia (VT) were reported. The resultant positive criteria of VLP were recommended with above method by using domestic instrument. Cut-off frequency effected on the results of analysis. The total QRS durations after high-pass filtering and the areas of the same frequent section have statistically significant differences between men and women (P<0.001). It is necessory for satisfactory signal -to-noise ratio, i. e., lower than 0. l~0.4μV,to collecting and averaging 200 QRS cycles. Further-more,the analysed section (A60) continuing 140ms from the point which is of 60ms after the beginning of QRS was a better onefor FD analysis. Our results showed that the false positive rate of VLP was 1.6% in N in the two analyses and that the positive rates of VLP by using TD analysis and FD analysis were 20% and 28% in MI patients, and 18% and 18% in VT patients respectively. However, the positive rates of VLP of 42% and 25% were showed by using FD analysis in MI patients with and without ventricular arrhythmia respectively.
